Work Authorization

I warrant and represent that I am the owner of the above described vehicle (hereinafter "the vehicle") or an authorized agent of the owner and that Collision Care Pros, LLC. is authorized to repair the vehicle and provide the necessary services, part, and materials needed for the repair. I further authorize Collision Care Pros, LLC. employees to operate the vehicle in streets and elsewhere for the purpose of testing and/or inspection at my own risk. I acknowledge and understand that Collision Care Pros, LLC. will have an express mechanics lien on and a security interest in the vehicle until such time as all indebtedness is paid in full and if I choose to cancel my repairs and reassembly a fee of up to $500.00 will apply in addition to any restocking or nonrefundable items. I further agree that Collision Care Pros, LLC., shall not be responsible for loss or damage to articles left in the vehicle in case of fire, theft, or any other cause. I further authorize Collision Care Pros, LLC. to be my representative to negotiate with any insurance company regarding the restoration of the vehicle to pre-accident condition and value. Collision Care Pros, LLC. is authorized to endorse drafts, checks or bills of exchange from insurance company securing payment as credit on my account for repairs on the above said vehicle. I appoint Collision Care Pros, LLC. as my Power of Attorney.
