Vehicle Details

Work Authorization

I warrant and represent that I am the owner of the above described vehicle (hereinafter "the vehicle") or an authorized agent of the owner and that Sook Paint & Body Shop LLC is authorized to repair the vehicle and provide the necessary services, part, and materials needed for the repair. I further authorize Sook Paint & Body Shop LLC employees to operate the vehicle in streets and elsewhere for the purpose of testing and/or inspection at my own risk. I acknowledge and understand that Sook Paint & Body Shop LLC will have an express mechanics lien on and a security interest in the vehicle until such time as all indebtedness is paid in full and if I choose to cancel my repairs and reassembly a fee of up to $500.00 will apply in addition to any restocking or nonrefundable items. I further agree that Sook Paint & Body Shop LLC, shall not be responsible for loss or damage to articles left in the vehicle in case of fire, theft, or any other cause. I further authorize Sook Paint & Body Shop LLC to be my representative to negotiate with any insurance company regarding the restoration of the vehicle to pre-accident condition and value. Sook Paint & Body Shop LLC is authorized to endorse drafts, checks or bills of exchange from insurance company securing payment as credit on my account for repairs on the above said vehicle. I appoint Sook Paint & Body Shop LLC as my Power of Attorney.
